JOB SUMMARY

The Integrated Care Therapist will provide mental health assessment, crisis intervention, safety planning, brief therapy (up to three months), care coordination and other services as part of the Mental Health staff who are integrated with the primary care teams. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

JOB RESPONSIBILITIES
  • Works with the Medical Department, Behavioral Health Department and others to provide crisis intervention, urgent care, risk assessment, mental health screenings, brief therapy, care coordination and warm handoffs.
MINIMUM REQUIREMENTS
  • Master’s degree in a behavioral health science field from an accredited university which meets the standards for Oregon licensure for LPC or LCSW.
PREFERRED REQUIREMENTS
  • Eight-plus years of managerial experience.
  • Eight-plus years of project management experience in Telecommunications or Energy related fields.
